Description
Facilitates safe, supported visitation and connection between infants and their parents or caregivers while providing comprehensive care coordination and referrals to strengthen families and mitigate the long-term effects of prenatal substance exposure. Services include supervised visitation, family bonding support, and connections to resources such as WIC, primary and dental care, postpartum and behavioral health services, substance use treatment including MAT and detox, housing assistance, clothing closet, child care, early intervention through ESIT, legal assistance, Family Team Decision Meetings (FTDM), and ID or driver's license support, creating a supportive environment that promotes family stability and infant well-being.
