Provides preventive health and education services to help individuals have a healthy pregnancy and a healthy baby. Provides social work counseling, nutritional counseling, community health counseling, and case management.
Services are offered to pregnant people enrolled in Medicaid in addition to medical and prenatal care and continue postpartum if the infant is enrolled in Medicaid until the infant's first birthday.
