Eligibility
Medicaid-eligible women who:
- Are currently pregnant or postpartum up to 24 months, prioritizing women who are pregnant and up to 12 months postpartum
- Self-report at risk substance use (drugs and/or alcohol) during the current or recent pregnancy.
- Have not successfully accessed community resources for substance use treatment and long-term recovery.

Provides three years of intensive case management to pregnant and parenting women with substance use disorders and their young children. Services include:
- Referral, support, and advocacy for substance use disorder treatment and continuing care.
- Assistance in accessing and using local resources such as family planning, safe housing, healthcare, domestic violence services, parent skills training, child welfare, childcare, transportation, and legal services.
- Connections to health care and therapeutic interventions for children.
