Eligibility
The eligibility requirement are the following:
- Maternity Support: Must be receiving a WA Apple Health (Medicaid), be within the pregnancy cycle, and accept the services.
- Infant Case Management: The baby must be on WA Apple Health (Medicaid), be 3-12 months, and meet specific criteria set by DSHS.
- Maternal Child Health: The child must meet risk criteria, be under 18 years of age, and reside in Yakima County.
- Maternity Case Management: Must be receiving on WA Apple Health (Medicaid), be within the pregnancy cycle, and accept the services.
