Konnect II offered by POCAAN

Helps and assists HIV+ individuals of color who are newly diagnosed, at-risk or not receiving care, as well as those seeking culturally relevant forms of emotional support and counseling.   Assists clients with referrals to health and social service resources, education, information and weekly support groups.   Brochures on HIV/AIDS are available in Spanish.   Also provides the following services:   -  Reentry support -  HIV testing -  Preventative education -  Recovery support groups   -  Case management
