Provides a program for families with children who have special health care needs. This service helps coordinate and provide access to identification, diagnosis, treatment and rehabilitation for children under 18 years of age.
Children with Special Health Care Needs (CSHCN) is a program that extends and improves access to medical services. Provides access for identification, diagnosis, treatment, and rehabilitation of children under the age of 18 with physically handicapping or potentially handicapping conditions. It reduces the financial impact these services have on families who lack adequate resources for care. Children with special health care needs may benefit from services such as: Early identification of health or developmental problems, Screening and/or assessment of the child and family's concerns, priorities and resources, Tracking or monitoring, Therapeutic intervention(s) including family education and support, and resource identification, referral, and coordination. Home visits and/or telephone consultations with a Community Health Nurse can help provide Care Coordination Services. And offer education about the following: Medical problems, the care needs and available resources Child behavior, growth and development, and Nutrition, basic baby and childcare Parenting issues Immunization information Safety issues Stress reduction Taking care of yourself to prevent illness Assist with problem solving and decision making

The nurse is able to help families navigate through complex medical systems and connect with resources.
Provides home or office nursing visits for assessment, referral and health education through the second month after the baby is born.

Participants in the CPR class receive a Medic II CPR card but not a certificate.

CPR class is intended for the general public for adult CPR and choking techniques.
Participants do receive a Medic II CPR card, good for 2 years. The card is not a certificate.
The newsletter 'Med Info' is published quarterly and lists classes and special events.

Hosts family networking & support events, specifically for the Down syndrome community. Works with individuals, families and caregivers to provide education and advocacy. Families needing information and education specifically related to Down syndrome are provided a trained Parent Advocate to address their needs. Families are also connected to other families to develop natural peer support.

- Family planning, genetic counseling, childbirth education (breastfeeding, baby basic, boot camp for new dads), pre-registration and childbirth planning, lactation consultants; Labor, Delivery, and Postpartum - private birthing suites, NICU, prenatal yoga.
- Cooking classes, Childhood Obesity Program-ACT, Free fitness classes
- Diabetes Wellness which offers insulin pump training, Continuous Glucose Monitoring, Diabetes Classes, one-on-one counseling, foot screenings and nail care, and a support group for English speakers.
- Online Childbirth Classes: support groups, breast feeding classes.
