Specially trained Community Health Nurses visit families in their home throughout the pregnancy and continue to visit until the child is 2 years old. The nurses visit weekly and bi-weekly during the pregnancy, infancy and toddler period. The education curriculum includes information during each phase of the development. This relationship based model builds on the parent’s strengths supporting the parent child relationship, healthy parent-child outcomes and goals for the future helping first time parents succeed. This program is free to eligible women.

Provides a partnership with registered nurses. Nurses do in-home visits with health and developmental screenings to identify potential problems with growth, hearing, social-emotional development, oral health, and other issues. Additionally, they may offer support with breastfeeding, parenting skills, and other general parent education. Visits will continue until the child reaches 2 years of age.

Promotes an integrated system of services for infants, children and youth up to age 18 years who have or are at risk for chronic physical, developmental, behavioral, or emotional conditions and require health and related services of a type or amount beyond what is generally needed. The nurse is able to help families navigate through complex medical systems and connect with resources. Provides home or office nursing visits for assessment, referral and health education through the second month after the baby is born.

Provides several services that promote healthy pregnancies and early childhood support. Black Infant Health Works with African American churches and Health Ministers to support pregnant and parenting mothers and their families. Provides health education and linkages with medical and community resources. Children with Special Health Care Needs - CSHCN Connects families of children with special health care needs to providers and services. Provides case management, care coordination and referral to community-based programs. Maternal Child Outreach Team - MCOT Works to reduce high rates of infant mortality in Pierce County. Connects newly pregnant mothers to health insurance including Medicaid, smoking cessation and other support services. Maternity Support Services - MSS Promotes healthy pregnancies and babies through nurse home visiting. Provides preventative health and education services to women who are enrolled in Medicaid. Nurse Family Partnership - NFP Provides nurse home visiting services for first-time pregnant low-income women. Improves birth outcomes, child health and development, and economic self-sufficiency of families. Referrals must be received by 26th week of pregnancy.

Program designed to help pregnant women receive the medical and social services they may need. All pregnant women covered under Medicaid (WA Apple Health) are eligible for services. Offers a team that helps advise and support mothers through their pregnancy. Team includes a: - Registered Nurse - Nutritionist - Behavioral Health Specialist - Community Health Worker - an Infant Case Manager. Services are provided wherever convenient for participants whether that be in home, clinic or in the community.
